The official figures for 2027
The Federal Office of Public Health (FOPH/BAG) published the 2027 premiums on 29 September 2026. The average premium includes all deductibles and models, which is why it is lower than the premium for the standard model with the lowest deductible.
| Age group | 2026 | 2027 | Change |
|---|---|---|---|
| Adults 26+ | CHF 464.80 | CHF 487.60 | +4.9 % |
| Young adults 19–25 | CHF 323.40 | CHF 338.80 | +4.8 % |
| Children 0–18 | CHF 122.40 | CHF 128.10 | +4.6 % |
| All insured | CHF 392.36 | CHF 412.06 | +5.0 % |
By canton, the average increase ranges from 3.0 % in Glarus to 6.6 % in Jura. Cantons such as Basel-Stadt, Geneva and Ticino have the highest premiums in absolute terms.
What determines your premium
Basic insurance (KVG/LAMal) is mandatory for everyone living in Switzerland and covers the same benefits at every insurer. What differs is the price. Five factors decide: canton and premium region, deductible (CHF 300 to CHF 2500 for adults), insurance model (standard, family doctor, HMO, telemedicine), accident cover and the insurer itself.
Newcomers have three months from arrival to take out insurance; cover is then backdated to the arrival date. Late registration can lead to a surcharge.
- Same benefits at every insurer
- Alternative models usually save 10–25 %
- Higher deductible = lower premium, higher risk
How to switch for 2027
Your insurer must send you your new premium by 31 October. To switch basic insurance for 1 January, your cancellation letter must arrive at your current insurer by 30 November – the postmark does not count. The FOPH recommends sending it by registered mail by mid-November. The new insurer must accept you in basic insurance without health questions.
Supplementary insurance (hospital, dental, alternative medicine) is a separate contract with health questions and its own notice periods. Never cancel it before a new one is confirmed in writing. Compare all insurers on priminfo.ch, the official and ad-free comparison tool of the federal government.
Deductible: the second bill
On top of the premium, you pay a share of your medical costs: first the deductible (franchise) of CHF 300 to 2,500 per year for adults, then 10 % of further costs up to CHF 700 per year, plus CHF 15 per day in hospital. Children have lower limits. In a bad year, an adult with the highest deductible pays up to CHF 3,200 out of pocket in addition to premiums.
Choosing a high deductible makes sense if you rarely see a doctor and have savings to cover it. If you have a chronic condition or expect treatment, a low deductible is usually cheaper overall. You can lower your deductible by 30 November and raise it by 31 December for the next year.
Budgeting the increase
For a couple with two children, the average increase adds up to around CHF 684 per year. If you raise your deductible to save on premiums, set aside the difference monthly so that a medical bill does not hit your budget unprepared. Low-income households can apply for a premium subsidy in their canton.
